Pre-Med to Doctor: The True Timeline, Cost, and Salary at Every Stage
From freshman year to attending physician: the real timeline (11-15 years), true cost ($300K+ debt), and salary at every stage of becoming a doctor.
The Longest Career Path in America
Becoming a doctor requires more education and training than almost any other career. Understanding the full timeline — and the financial reality at each stage — is essential before committing to pre-med.
Undergraduate (4 years) — You Pay
Cost: $40K-$300K depending on school
Complete pre-med requirements (biology, chemistry, organic chemistry, physics, biochemistry) while maintaining a 3.7+ GPA. Take the MCAT junior year. Any major works, but you must excel in the sciences.
Medical School (4 years) — You Pay a Lot
Cost: $200K-$350K total
First 2 years: classroom and lab. Last 2 years: clinical rotations in hospitals. Average debt at graduation: $200K-$250K.
Residency (3-7 years) — You Get Paid Modestly
Salary: $60K-$75K
60-80 hour weeks. You're a doctor, but you're still training. Length depends on specialty: family medicine (3 years), surgery (5-7 years), neurosurgery (7 years).
Fellowship (1-3 years, optional)
Salary: $70K-$90K
Sub-specialize further. Cardiology, oncology, sports medicine, etc. Adds income potential but delays your attending salary.
Attending Physician — The Payoff
- Family Medicine: $230K-$280K
- Internal Medicine: $260K-$310K
- Emergency Medicine: $310K-$380K
- Anesthesiology: $380K-$450K
- Orthopedic Surgery: $500K-$700K
- Neurosurgery: $600K-$800K+
The Math Nobody Does
A primary care physician doesn't start earning real money until age 30-33. An orthopedic surgeon might be 35-37. Factor in $250K+ in debt with interest, and the lifetime earnings comparison with other high-paying careers (engineering, tech, finance) is closer than most people think. Medicine is a calling — make sure it's yours before committing 11-15 years.
